Position Description
Job Title: RN Home Health and Hospice
Location: Saint Johnsbury, Vermont, 05819
Contract duration: 8 weeks
Shift:5X8-hours
Schedule: Days
Guaranteed Hours: 40/week
Key Responsibilities:
- Perform initial and ongoing patient/family assessments to determine care needs.
- Provide complete physical assessments and health histories.
- Develop, implement, and regularly update individualized plans of care.
- Deliver therapeutic, preventive, and rehabilitative nursing care.
- Administer prescribed medications and treatments.
- Provide patient and family education and counseling.
- Identify and coordinate discharge planning needs.
- Act as Case Manager when assigned, coordinating care for a caseload of patients.
Communication
- Document clinical notes accurately and in a timely manner.
- Communicate effectively with physicians regarding patient status, care needs, and plan of care updates.
- Obtain and follow physician orders as required.
Additional Duties
- Participate in on-call rotation and holiday coverage.
- Coordinate equipment, supplies, and additional services as needed.
- Supervise, instruct, and evaluate home health aide care per state and federal regulations.
- Perform other job-related tasks as assigned.
Requirements:
- RN License
- BLS
- 1–2 years of acute care or clinical experience preferred (home health or hospice experience a plus).
- Strong clinical, assessment, and critical thinking skills.
- Excellent written, verbal, and interpersonal communication skills.
- Valid driver’s license, reliable automobile, and proof of insurance.
